State Health Requirements

Please check the chart below to verify which vaccinations your child will need for the upcoming school year. All health documents must be returned to your child’s school nurse prior to the first day of school by mail, email, or fax to the appropriate building.

• Vision Screening Form
  • Required for Kindergarten and 3rd grade
• Dental Screening Form
  • Required for Kindergarten and 9th grade
• Immunization Certificate
  • All students must have an up to date certificate
    • Preschool: Immunizations current for age
    • Kindergarten: Immunizations current for age
    • 7th grade: Tdap booster and meningococcal vaccine requirement
    • 12th grade: Booster for meningococcal vaccine
  • Exemptions 
    • Religious
    • Medical
• Lead Screening
  • Required for kindergarten
  • There is no form available, but it will be cross-referenced with your healthcare provider
• Preschool Physical Form & Kindergarten Physical Form
  • Required for Preschool & Kindergarten
• Physical Form

Does your child need to take medication during the school day? Please complete the Authorization to Administer Medication Form and return it to your child’s school nurse prior to the first day of school.

• MMR - 1 dose
• Polio - 3 doses
• Varicella - 1 dose
• HIB - 3 doses

Kindergarten

• Dental screen
• Vision screen
• Lead screen
• DTap - 5th dose
• MMR - 2 doses
• Polio - 4th dose
• Varicella - 2 doses
• HepB - 3 doses

3rd Grade

• Vision Screen

7th Grade

• Tdap
• Meningitis - 1 dose

9th Grade
